Our service supports assessment, rehabilitation, mobility, pain management and falls prevention, with all clinical recommendations documented directly in your care plans.
Our physiotherapy service
Our Chartered Physiotherapists provide assessment, treatment and rehabilitation for residents in nursing homes, hospitals and community healthcare settings across Ireland. Working alongside your nursing and care teams, they develop individual treatment plans that support mobility, rehabilitation, pain management and falls prevention.
The service gives healthcare providers access to regular physiotherapy support without employing a full-time Physiotherapist. Clinical recommendations are documented within your care plans, while staff receive practical guidance to help residents continue their programmes between visits.
How physiotherapy is delivered
Physiotherapy is provided on an agreed schedule, from a regular weekly session to a set number of days each month, depending on your requirements.
During each visit your Physiotherapist assesses residents, delivers treatment, reviews progress and updates care plans. Programmes are adapted as residents’ needs change, with regular reviews helping to maintain continuity of care and support long-term rehabilitation.

Structured physiotherapy
Our physiotherapy service is delivered through the Active Living Programme, a structured approach that combines assessment, treatment, staff education and regular review. Each stage builds on the previous one, helping residents progress while supporting continuity of care within your service.
Stage 1
New and existing residents receive a comprehensive physiotherapy assessment covering mobility, function, falls risk and manual handling requirements. Individual treatment plans, clinical recommendations and manual handling reports are completed where appropriate.
Stage 2
Residents receive physiotherapy tailored to their individual needs, including rehabilitation, exercise programmes, pain management and education relating to their condition. Treatment plans are reviewed and updated as residents progress or their needs change.
Stage 3
Your Physiotherapist works alongside your care team, providing practical guidance on transfers, mobilisation, rehabilitation, contracture management, bone health and osteoporosis. This helps staff continue each resident’s programme safely and consistently between visits.
Stage 4
Residents take part in individual and group exercise sessions designed to improve balance, coordination, posture, mobility and cardiovascular fitness while supporting rehabilitation, reducing falls risk and promoting independence.
Stage 5
Resident progress is reviewed regularly, with structured sign-in and sign-out handovers completed during each visit. Clinical documentation is maintained within your care plans to support continuity of care, clinical governance and regulatory requirements.
